How the Social Care Network works.

We are part of a group of South Bronx organizations that connect Medicaid members to help with health-related social needs.

  1. Screen

We look at housing, nutrition, transportation, employment, education and personal safety.

  2. Navigate

We match each need to federal, state and local programs that fit.

  3. Receive

We coordinate one-time or up to six months of back rent and utilities, meals, transportation and more.

Working alongside trusted partners.

Our work is made possible by foundations, corporations, member organizations and donors.
