Behavioral health, housing, employment and the paperwork in between, handled by people who know your full situation.
Mental health services are offered in individual and group settings, for clients of all ages, alongside our other programs.
Our mental health and therapeutic team focuses on social skills, communication, independence, healthy habits and more.

We look at how mental health shapes your well-being, motivation and outlook on life, and we treat the whole person.
Many families face real barriers to care. Our mission is to break those barriers down with quality counseling, genuine care and psychoeducation.
Our Employment Services Program helps you pursue the right job opportunities with confidence.
An assigned employment coach helps define your goals and assess your situation, then supports you as you prepare and apply.

We help you obtain an appropriate housing placement. We aim to secure apartments, though sometimes it may be a shared room or a Single Room Occupancy (SRO).
Many people struggle to keep stable housing without support for unexpected challenges, so we stay with newly housed clients as they settle into a new home.

Each client gets individual help to submit the application, then help finding an agency that will accept it once approved.
Every client completes a housing assessment with a member of our clinical team.
Ongoing individual support, and help to secure low income housing once the subsidy is no longer available.
Alongside counseling, housing and employment, we coordinate medical care and help with the applications that keep everything on track.
Call us Monday to Friday, 9 AM to 5 PM. We will listen first, then point you to the right help.
The Social Care Network, step by step.
We are part of a group of local, mostly non-profit organizations supporting health and well-being across the South Bronx. Together we connect Medicaid members to services for health-related social needs.

We coordinate one-time or up to six months of back rent and utilities, medically tailored meals, transportation and other services.
About the program
Social Care Networks are a New York State Medicaid program, created under the New York Health Equity Reform 1115 waiver approved in January 2024. Nine regional networks connect Medicaid members to free services for housing, food, transportation and more, delivered by local non-profits.
In the Bronx, the network is led by SOMOS Community Care. We work with SOMOS to screen our clients and connect them to services.
Get screened with us
A member of our team screens you, then handles the connection to services for you. Call us Monday to Friday, 9 AM to 5 PM, or send a message.
The goal is better social and health outcomes, by addressing the needs that sit beyond traditional health care.
